  1. 1

    Clinical license and certification

LCSW, CCM, the emergency department and complex discharges. The clinical license and the case management certification together tell a hospital this person can take its hardest cases.

  2. 2

    Avoidable days with a before

Avoidable days from 1,900 to 1,150 a year. Every day a ready patient waits costs the hospital a bed, so the before and after is the number a director reads first.

  3. 3

    What caused the waits

Guardianship, placement and authorization, with referrals and court petitions started earlier. Naming the causes shows the case manager understood exactly why ready patients were stuck in beds.

  4. 4

    Admissions avoided safely

About 260 emergency patients a year sent home with services instead of admitted. Arranging care from the emergency department frees beds and keeps patients out of the hospital.

  5. 5

    Readmissions for frequent users

Readmissions for 45 frequent patients from 38% to 24%. A small group of frequent users drives many returns, so a care plan that lowers their rate is a result worth a line.

03The same resume at three levels

The tinted row is the level shown above.

  • Entry-level

    0–2 years

Education sits above the jobs, and the headline gives the license, the state, the units and the caseload. Bullets cover assessments, on-time discharges, placements and social needs. The internship folds to one line with its hours and the services it covered.

Typical titles: Social work case manager, Medical social worker, Hospital case manager

  • Mid-level

    3–6 years

The headline names the clinical license, the certification and the setting. Bullets give avoidable days with their causes, admissions avoided, goals-of-care conversations and readmissions for frequent patients. The health center years fold to one line with the caseload and the year the LCSW was earned.

Typical titles: Social work case manager, Emergency department case manager, Clinical social worker

  • Senior

    7+ years

Scope replaces caseload: the size of the team, the units and admissions covered, length of stay, avoidable days with the partnerships behind them, and discharges before noon. The six case manager years fold to one line with the daily caseload and the year the LCSW was earned.

Typical titles: Manager of social work, Care transitions manager, Director of case management

04Summary

Weak

Compassionate and organized case manager dedicated to advocating for patients and coordinating seamless care across the continuum.

Strong

Licensed clinical social worker with six years in health care, the last four as the emergency department case manager for a 420-bed hospital. Cut avoidable days for complex patients from 1,900 to 1,150 a year, and leads goals-of-care conversations with the palliative team.

The license, the setting, the hospital's size, a before and after on avoidable days and a clinical role. A director can compare each with their own team. The weak version promises advocacy without a license, a setting or a number.

05Bullets

Weak

  • Responsible for discharge planning and coordinating care for patients with complex needs.

Strong

  • Reduced avoidable days for patients waiting on guardianship, placement or authorization from 1,900 to 1,150 a year by starting referrals and court petitions earlier.

The causes of delay, a before and after, the time frame and the change that worked. A director can ask which cause moved most and how petitions were sped up. The weak bullet describes what every case manager is hired to do, with no setting, no number and no result, so every applicant in the pile could send it unchanged.

07Mistakes to avoid

  1. No license or setting named

Fix: Hospital, emergency department, community health and insurer case management are different jobs, and postings ask for a state license by name. Put your license, its state and your setting in the headline, so a manager can match you to the role at a glance.

  2. Readmission claims with no base

Fix: Reduced readmissions by 20% leaves the reader asking from what and for whom. Give the rate before and after, the patient group and the period, such as readmissions for 45 frequent patients from 38% to 24% in a year.

  3. Caseload with no outcome

Fix: A caseload number shows volume, not results. Pair it with what the hospital measures: discharges on the expected date, avoidable days, placements made, readmissions or discharges before noon, each with the count behind it.

  4. Duties instead of decisions

Fix: Responsible for discharge planning describes the job title. Show the judgment instead: the barrier you found, such as guardianship or authorization, the step you took, and how many days or admissions it saved.

  5. Clinical skills hidden

Fix: Crisis work, grief support and goals-of-care conversations are part of hospital social work, and practitioners argue they should be used at the top of the license. Name them with counts, so a reader sees more than logistics.

08Format and template

One column, reverse-chronological order and standard section headings, with every figure written as text. Put your license, its state and your setting in the headline. Give the measures your hospital tracks, such as discharges on time, avoidable days and readmissions. List each license and certification with its issuer and date. This example uses Green Academic. The same content works in Standard, Warm Classic and Beige Border.

09Questions case managers ask

Should a case manager list LSW, LMSW or LCSW on a resume?

List the exact license your state issues, with the state, in the headline and in a certifications section with its date. All states require clinical social workers to be licensed, and the clinical license follows a master's degree and supervised experience after graduation. Licensing boards use ASWB exams, a four-hour test of 122 questions, so mention when you passed if you are newly licensed.

What numbers should a hospital case manager put on a resume?

Use the measures your hospital keeps. Give your caseload and units, assessments completed on time, discharges on the expected date, placements made and avoidable days. Emergency department case managers should add admissions avoided and readmissions for frequent patients. Managers should add length of stay, discharges before noon and staff turnover. Always show the starting point beside a change, so a reader can judge its size.

Is the CCM worth getting for a social work case manager?

Often, yes. The Commission for Case Manager Certification calls the CCM a board certification for case managers, and many hospital postings say case management certification is desired or preferred, especially for senior roles. It requires case management experience before the exam, so list the date you expect to sit for it if you are working toward it, and the date earned once you hold it.

How should a new MSW graduate write a case manager resume?

Put your MSW first, then your field placements with their hours and the services you worked in, such as emergency, oncology or medical-surgical units. List the license you hold or the date you expect it. Some hospital postings let a field placement in their own care coordination department count toward the experience they ask for, so name where you placed and what you did there.

Should a case manager include clinical work like crisis or grief support?

Yes. Healthcare social workers help patients adjust to a diagnosis, and hospital postings list crisis intervention, grief counseling and goals-of-care support beside discharge planning. A practitioner writing for social work trade press argues that hospital social workers improve outcomes when they practice at the top of their license, so give counts for clinical work as well as placements and discharges.

How do I move from case manager to a manager of social work or care transitions?

Show the parts of management you already do. Supervising interns, leading discharge huddles, building partnerships with skilled nursing facilities and fixing a cause of delay all count. Give numbers for each. Manager postings ask for case management experience and leadership, and hospitals judge the role on length of stay, avoidable days and readmissions, so show any of those you have moved.
